During your first few visits, you may meet with a financial counselor to review your insurance information and discuss your insurance coverage. This allows us to file claims for all office visits and in-office procedures on your behalf and send out monthly statements to keep you informed. If you do not have insurance, the financial counselor will work with you to set up a payment schedule and provide you with information about agencies that offer assistance.

Our practice is open to all patients regardless of insurance. However, many patients in the Sacramento area are covered under a managed care plan, which may restrict access to physicians in their network. If you choose a physician out of your network, you become responsible for a larger share of your treatment costs.

Insurance plans and prepaid health plans vary greatly in their requirements for benefit authorization. We will assist you in meeting these requirements, but it is your responsibility to know your plan requirements and see that these requirements are met. Please contact your plan administrator if you have any questions about what services are covered by your insurance.

M e d i c a r e   a n d   M e d i - C a l
We accept Medicare assignment and bill most secondary insurances on your behalf. Medi-Cal patients are required to provide us a copy of their Medi-Cal card each month, and payment of Medi-Cal co-pays are due at the time of service.

C o - P a y s   a n d   D e d u c t i b l e s
All co-pays and deductibles are due at the time of service. Past due balances for private pay patients will be collected at your next visit.

N o t i f i c a t i o n   o f   C h a n g e s
If you change insurance or your primary care physician, please call us so that we can initiate the benefit authorization process. Please notify the receptionist at your next visit as well. If proper authorization is not received for your care after such a change, you become responsible for payment.
